What do I need to bring to my appointment?

When visiting your physician's office, bring your health identification card and co-payment, if applicable.

We ask that you pay all applicable co-payments at the time services are rendered. You will receive a bill if the services provided are not covered under your health plan.

Please arrive at least 15 minutes prior to your first scheduled appointment time to complete paperwork and give us a chance to become acquainted. We encourage you to make an appointment with your doctor immediately to determine your personal health care needs.

We also encourage you to print out the new patient forms ahead of time and complete them at your leisure prior to your first scheduled appointment. This will help us to serve you promptly.
