Illinois College Student Immunization Act
Please contact us to schedule your College Student Immunization appointment and comply with the Illinois College Student Immunization Act. Please indicate a preferred time, Wednesday morning 9-12noon or afternoon 2-5pm. Also provide your complete contact information and indicate your health insurance coverage, please indicate self pay if you are not covered. Note: This may not be covered under your insurance plan, in that case, we will let you know what you would be expected to pay out of pocket.

All students are required by Illinois State law (Illinois Administrative Code 694.100) and University regulations to provide proof of immunizations, in English. Proof of immunizations along with the Admission Health Record (AHR) must be submitted to the Health Service - Evanston Campus prior to entering a full-time or half-time academic program. Acceptable forms of documentation include (at least one of the following):
- PREFERRED -- Recording of vaccination dates by a licensed healthcare professional (e.g. physician, nurse, etc.) in the immunization section of the AHR.
- Copies of immunization records from a healthcare professional, previous school (must have healthcare professional signature).
- A copy of an official vaccination record (must have healthcare professional signature).
- Copy of a lab report confirming immunity is acceptable in place of vaccinations for: Measles, Mumps, Rubella, Hepatitis B, and Varicella.
| Measles (Rubeola) | Two vaccinations administered after first birthday, at least 28 days apart, and after 1/1/1968 OR physician's documentation of illness and date of diagnosis. |
| Mumps (Parotitis) | One vaccination administered after first birthday, and after 1/1/1968 OR physician's documentation of illness and date of diagnosis. |
| Rubella (German Measles) | One vaccination administered after first birthday, and after 1/1/1969. |

| Tetanus/Diphtheria Booster (DT) | One booster vaccination administered within 10 years of entrance into Northwestern. |

| Tuberculosis (TB) Self-Screening | Students not entering a healthcare program (e.g. Medical, Prosthetics and Orthotics, Physical Therapy, Genetic Counseling, or Graduate-Level Communication Sciences and Disorders programs) must complete the TB self-screening questionnaire (included in Admission Health Record). |

| Tuberculosis Testing (if required either by academic program or by TB Self-Screening) | Acceptable TB tests include: PPD skin test or a QuantiFERON® TB Gold blood test. You must submit either the PPD (including date placed, date read, results in mm induration, with a healthcare provider's signature), or a copy of a QuantiFERON® TB Gold blood test report. Either test must be done in the USA and within 6 months prior to entrance into Northwestern. |
| Hepatitis B | Three vaccination dates and a copy of a lab report confirming immunity. Vaccine dates alone are not sufficient. |
| Varicella (Chicken Pox) | Two vaccinations administered at least 28 days apart OR physician's documentation of illness and date of diagnosis. |
